Inserting an Icon using PBForms 1.5

I am having trouble inserting an icon into a dialog using PBForms 1.5.
I can get the icon to appear where the elipse is (...) but it does
not appear to save. If I go away from that dialog and then return
it is no longer there. Neither does it appear as the icon for the
program. Any help for this so called easy method of inserting an
icon would be gratefully received.
