Button With Text And Picture

Does anyone know (or better yet have some source code) how to
make a button with both a text caption and a picture image.
Would love to use DDT but what ever works.

Has anyone ever use Sheridan Command button from Sheridan Active
ThreeD Plus package with DDT? I know that allows both a caption
and an image on a button.
